Responsibilities
- Deliver personalized patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility
- Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates
- Support patients with medical procedures under supervision, including catheter care and basic wound management
- Maintain strict adherence to HIPAA regulations and ensure patient confidentiality at all times
- Document patient information precisely using Electronic Health Record (EHR) systems and medical documentation protocols
- Assist with patient transfers utilizing safe lifting practices and equipment such as Hoyer lifts or transfer belts
- Collaborate effectively with nursing staff, physicians, therapists, and family members to coordinate comprehensive care plans
Skills
- Proven experience working with diverse patient populations in settings such as long-term care facilities, hospitals, assisted living, or home care environments
- Strong knowledge of medical terminology, anatomy, physiology, and aseptic techniques necessary for safe patient care
- Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation and data management
- Ability to perform vital signs measurement accurately and recognize signs of patient distress or deterioration
- Experience working with individuals with developmental disabilities, dementia (including Alzheimer’s and memory care), or behavioral health needs
- Skilled in safe manual handling techniques and use of assistive devices such as Hoyer lifts or patient transfer methods
- Knowledge of infection control standards and procedures for infectious disease care and hospital environments including ICU or trauma center experience
