Another Day Life Home Health Care is seeking an experienced Registered Nurse (RN) with case management skills to serve clients in the Kansas City, MO area.
We are looking for an RN who understands the coordination of care and is comfortable working with clients receiving services through Missouri Medicaid/MO HealthNet and Home and Community Based Services (HCBS). The ideal candidate will have experience with client assessments, care planning, documentation, coordination of services, and ongoing monitoring of client needs.
Responsibilities
Provide nursing oversight and care management for assigned clients.
Complete client assessments and ongoing evaluations as required.
Monitor changes in client condition and service needs.
Maintain accurate and timely documentation.
Communicate changes or concerns that may require updates to client services or care plans.
Qualifications
Current Registered Nurse license in Missouri
Case management skills
Experience with Missouri Medicaid/MO HealthNet, HCBS, in-home care, or community-based services strongly preferred
Experience with client assessment, care plans, service coordination, and clinical documentation
Strong organizational and communication skills
Experience working with elderly individuals, ind with disabilities, and/or individuals with medically complex conditions preferred
