Job Overview
We are seeking a dedicated PRN Nurses LPN/RN to join our healthcare team. This role offers flexible scheduling to support our residents' needs across various shifts. The ideal candidate will possess a strong background in diverse nursing practices, including critical care, long-term care, and acute medical settings, with a focus on delivering compassionate patient-centered care.
Duties
- As a LPN/RN you will need to perform comprehensive patient assessments, vital sign monitoring, and physical examinations to inform individualized care plans.
- Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and medical documentation standards.
- Provide specialized care for residents with dementia, Alzheimer's disease, and memory impairments, including memory and behavioral health management.
- As a LPN/RN you will Assist with IV infusions, catheterization, tube feeding, Wound Vacs
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care services.
- Ensure infection control procedures are followed diligently to maintain a safe environment for residents and staff.
- Document all patient interactions thoroughly using EMR/EHR systems such as Point Click Care; maintain clinical confidentiality at all times.
Qualifications
- Valid LPN or RN licensure in good standing; additional certifications such as BLS/CPR required.
- Demonstrated experience working in diverse healthcare environments including hospital medicine, long-term care, assisted living, or skilled nursing facilities.
- Proficiency with EMR/EHR systems like Point Click Care; familiarity with ICD-9/10 coding is preferred.
- Strong knowledge of geriatric care principles, dementia care strategies, and acute pain management techniques.
- Experience working with individuals with developmental disabilities or behavioral health needs is advantageous.
- Ability to perform venipuncture, sterile procedures, and basic life support confidently.
- Excellent communication skills for effective caregiver communication and patient assessment; compassionate approach essential.
