Wakefield Health Care Center is seeking an experienced LPN for a travel Long-Term Care (LTC) assignment. The facility serves primarily LTC and dementia residents, with approximately 36 SNF and 14 ALF residents.
Key Responsibilities
- Provide direct nursing care to LTC and dementia residents.
- Administer medications and treatments according to physician orders.
- Monitor resident conditions and report changes appropriately.
- Complete accurate documentation in PointClickCare.
- Collaborate with CNAs and interdisciplinary staff.
- Maintain patient safety and infection-control standards.
- Support residents across East and West halls.
Requirements
- Active Nebraska LPN license
- LTC/SNF experience preferred
- Willingness to work 12-hour night shifts
- Weekend availability required
- Comfortable with approximately 1:20 nurse-to-patient ratio
