Responsibilities
- Conduct comprehensive patient observations to monitor health status and identify changes promptly
- Assist with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support
- Administer medications accurately following prescribed medical protocols and document all medication records
- Monitor vital signs including blood pressure, pulse, temperature, and respiration rates to assess patient health
- Perform specialized care tasks such as catheter care, wound management, and assisting with medical procedures using EMR (Electronic Medical Record) systems
- Support patients with dementia or Alzheimer’s by implementing memory care strategies and providing compassionate companionship
- Ensure compliance with home healthcare regulations and maintain patient confidentiality in accordance with HIPAA standards
- Utilize patient transfer methods safely using equipment like Hoyer lifts or other assistive devices when necessary
- Engage in patient education regarding health maintenance, medication adherence, and safety precautions
- Collaborate effectively with family members and healthcare teams to coordinate ongoing care plans
Qualifications
- Valid nursing license (RN or LPN/LVN) in good standing with relevant state licensing boards
- Proven experience in home healthcare settings or assisted living facilities preferred
- Strong knowledge of medical terminology, medication administration, and vital signs monitoring
- Experience working with individuals with disabilities or developmental challenges highly desirable
- Familiarity with EMR/EHR systems for accurate documentation of patient records and medical history
- Ability to perform patient personal care assistance safely and compassionately
- Knowledge of hospice care, geriatrics, dementia care, and end-of-life support is a plus
- Certifications such as CPR (Cardiopulmonary Resuscitation) and First Aid required; Basic Life Support (BLS) certification preferred
- Physical ability to lift heavy loads safely and perform patient transfers using appropriate techniques and equipment
- Excellent communication skills for client interaction and team collaboration
