Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Resident Care Coordinator (RCC) to join our dynamic healthcare team. Responsibilities
- Deliver comprehensive nursing care, including medication administration, vital signs monitoring, and physical assessments
- Develop, implement, and update personalized care plans in collaboration with interdisciplinary teams
- Coordinate resident activities related to health management, catheter care, and wound management
- Conduct triage assessments to determine urgency and appropriate interventions for residents’ health concerns
- Maintain accurate medical documentation using electronic health record (EHR) systems such as Epic or Cerner
- Ensure compliance with HIPAA regulations and uphold patient confidentiality at all times
- Provide education and support to residents and their families regarding health conditions, medications, and treatment options
- Assist with discharge planning, post-acute care coordination, and referrals to specialists when necessary
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent certification
- Proven experience in long-term care, assisted living, or nursing home environments
- Strong knowledge of medical terminology, anatomy, and physiology
- Experience with EMR/EHR systems preferred
- Skills in patient assessment, medication administration, catheterization, and sterile techniques
- Ability to work effectively with individuals with dementia-related conditions such as Alzheimer’s disease or memory care needs
- Excellent communication skills
- Knowledge of infection control protocols and safety procedures in healthcare environments
