Case Manager Morris
Horizon Public Health is seeking applicants for a Full-Time Registered Nurse to provide case management services out of our Morris office. A Case Manager provides care coordination, including developing, implementing and evaluating client plan of care, coordinating care using available resources and maintaining contact with care givers. Specifically, this position will be working with clients under age 65.
Minimum qualifications include a Baccalaureate degree in Nursing and licensed in MN with at least one year of home and community-based experience. Exceptions may apply for ADN applicants with at least two years of home and community-based experience. Prior experience in long term care is also beneficial. Successful applicants must pass a criminal background check and a pre-employment drug test. Must have valid MN driver’s license and reliable, insured transportation. This is a Grade 13 MNA/AFSCME Council #65 union position.
Starting salary range is $36.03-$38.23/hour with earning potential up to $47.01/hour. Addendum – Case Manager
Program Responsibilities
Assist with planning, developing, implementing and evaluating assigned Public Health Programs.
Conduct at least bi-annual visits and annual reassessments, as well as unscheduled reassessments (for change in condition) on all clients to determine level of care, program eligibility and review the services needs and plan of care.
Develop an evidence based, patient centered and goal directed plan of care that is specific to each individual client. Include the patient and family in developing the care plan and update as needed using available resources. Maintain contact with family or informal care givers as appropriate.
Assist clients in establishing care with medical and mental health providers. Encourage preventative health visits and immunizations to prevent disease, monitor health status and attend medical appointments with the client as needed.
Provide resources/referrals for mentally ill clients in a crisis situation. Assist and educate on problem solving and working with challenging behaviors of mentally ill clients and their families.
Maintain privacy of the clients we serve along with maintaining appropriate professional boundaries.
Coordinate client transfers into and out of the county, maintaining communication with the financial workers and case managers, assuring that the health and safety needs of the client are met.
Provide teaching on chronic disease management to client, their family, legal guardian and provides to promote optimal health and wellbeing along with assisting the client in accessing services and providers. Educating the client, family/or legal representative, in their rights and responsibilities.
Complete and maintain documentation of individual client records and waiver program activities per agency requirements.
Follows Managed Care Organization directives and guidelines.
Collaborates with providers to manage and assure vulnerable population needs are evaluated and effective community supports plans are formulated and instituted.
Research and possess knowledge of multiple providers and services available to clients.
Preform individual and community assessments. Provide health information and education of relevant health topics as well as prevention of injury and disease. Promote healthy behavior in individual, families and groups.
Research and monitor state and federal regulations relevant to assigned programs.
Work with clients to develop and implement emergency preparedness planning.
Formulate budgets for eligible clients as appropriate within their program guidelines.
Attend meetings, workshops and training sessions appropriate for program and licensure.
Actively participates in discharge planning.
Other program responsibilities as outlined by supervisor.
