Bradford Hills is seeking a Full-Time Occupational Therapist (OT) to join our skilled nursing and long-term care (SNF/LTC) team in West Burlington, PA. This is a great opportunity for an OT who is passionate about patient-centered care and wants to work in a supportive, collaborative environment.
Full-time, consistent caseload
SNF/LTC setting
Collaborative interdisciplinary team
Modern documentation systems (Net Health & PCC)
Evaluate residents and develop individualized treatment plans
Provide occupational therapy services to improve functional independence
Document evaluations, progress notes, and discharge summaries accurately and timely
Collaborate with nursing, rehab, and medical staff to ensure high-quality care
Educate residents and families on treatment goals and progress
Licensed Occupational Therapist in the state of Pennsylvania
Experience in SNF/LTC preferred (new grads encouraged to apply)
Strong communication and organizational skills
Familiarity with Net Health and PCC a plus
Supportive leadership and rehab team
