Physical Therapist (PT) – Home Health – Flexible Schedule
Location: Central/South Orange County

What You’ll Do:
- Deliver 1:1 skilled physical therapy care to patients in their homes—no clinic walls, just real connection.
- Perform comprehensive physical therapy evaluations and establish individualized, evidence-based care plans.
- Provide hands-on therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
- Monitor and document patient progress, outcomes, and response to treatment, adjusting the plan of care as clinically indicated.
- Educate patients and caregivers on home exercise programs, safety strategies, and mobility techniques to promote carryover and prevent injury.
- Collaborate closely with a multidisciplinary team including nurses, occupational therapists, speech therapists, and physicians.
- Maintain a safe, respectful, and professional home care environment that supports patient dignity and comfort.
What You Bring:
- Licensed PT CA (required)
- Strong communication skills and a compassionate approach to care
- Home health experience is a plus, but not required
- Familiarity with orthopedic and/or geriatric populations a bonus
- Ability to work independently—and still be a team player
- Reliable transportation for home visits

Pay: $75.00 - $120.00 per hour
