Premier Physical Therapy is urgently seeking an experienced Speech-Language Pathologist to provide ongoing, one-on-one services for a VIP adult patient in his Wellington home.

Premier already provides the patient’s physical therapy team. The selected SLP will work collaboratively with our PTs as part of the patient’s long-term neurological rehabilitation plan.
Patient Overview
The patient is recovering from a brain bleed and was recently discharged home after receiving care in the ICU, inpatient rehabilitation and skilled rehabilitation. He is primarily wheelchair-dependent, has no functional use of one arm and requires assistance with transfers. He can bathe and use the restroom independently.
Responsibilities
- Complete a comprehensive in-home speech-language evaluation
- Evaluate speech, language and cognitive-communication abilities
- Evaluate swallowing function when clinically indicated
- Develop and implement an individualized neurological treatment plan
- Address aphasia, apraxia, dysarthria and cognitive impairment as applicable
- Support functional communication, attention, memory and problem-solving
- Provide swallowing treatment and safety recommendations when appropriate
- Educate the patient, caregivers and Premier’s therapy team
- Coordinate care with the patient’s existing physical therapists
- Complete accurate and timely clinical documentation
Qualifications
- Active, unrestricted Florida Speech-Language Pathologist license
- ASHA Certificate of Clinical Competence (CCC-SLP) strongly preferred
- Adult neurological, stroke or brain-injury experience strongly preferred
- Experience evaluating or treating dysphagia preferred
- Home-health, acute-care, inpatient rehabilitation or skilled-nursing experience preferred
- Ability to travel to the patient’s home in Wellington
- Professional liability insurance
- Dependable communication and timely documentation
- Ability to work independently and collaboratively
Schedule and Compensation
This is a flexible PRN independent-contractor opportunity dedicated to one patient. Visit frequency, scheduling and compensation will be discussed based on experience and availability.
