Continental Cozy Healthcare is seeking a motivated and compassionate Community Outreach & Referral Marketing Coordinator to increase community awareness, build strategic partnerships, and generate referrals for our services. This individual will represent the agency throughout Washington, DC, and surrounding communities by connecting with healthcare professionals, schools, community organizations, and families to promote person-centered services for individuals with intellectual and developmental disabilities.
Key Responsibilities
* Promote Continental Cozy Healthcare services throughout Washington, DC and surrounding communities.
* Build and maintain relationships with Service Coordinators, case managers, social workers, hospitals, physicians, schools, rehabilitation centers, churches, and community organizations.
* Conduct outreach visits and presentations to referral partners and community organizations.
* Represent the agency at health fairs, community events, networking meetings, and conferences.
* Educate individuals, families, guardians, and caregivers about available services and supports.
* Identify, develop, and maintain referral sources to increase client enrollment.
* Distribute marketing materials and assist with community engagement and marketing initiatives.
* Maintain accurate records of outreach activities, referrals, and community partnerships.
* Follow up promptly with referral sources and prospective clients.
* Prepare outreach reports and collaborate with leadership to achieve referral and growth goals.
* Represent the agency professionally while upholding its mission, vision, and values.
Qualifications
* High school diploma required; Associate’s or Bachelor’s degree preferred.
* Experience in community outreach, healthcare, marketing, human services, or social services preferred.
* Excellent communication, networking, and relationship-building skills.
* Strong organizational and time-management abilities.
* Ability to work independently and meet outreach goals.
* Proficiency with Microsoft Office, email, and social media platforms.
* Reliable transportation and willingness to travel throughout the community.
Preferred Experience
* Knowledge of DDS services, HCBS Waiver programs, Supported Living, Host Home, In-Home Supports, Companion Services, Respite, or Employment Services.
* Experience working with individuals with intellectual and developmental disabilities and their families.
* Existing relationships with community organizations and referral sources.
