Position Summary
The will support the Clinical Care Team by addressing patients’ social and healthcare needs, coordinating community resources, and providing case management services. This role focuses on improving access to care by assisting with social determinants of health, referrals, discharge planning, and patient education.
Key Responsibilities
- Conduct social needs screenings and connect patients to community resources (housing, food, mental health, etc.)
- Provide case management, follow-ups, and discharge coordination
- Assist with specialty referrals, appointment scheduling, and medical records
- Support patients with low-cost medication programs and financial assistance applications
- Maintain documentation in patient records and collaborate with providers
- Participate in clinical and community meetings to strengthen local partnerships
Qualifications
- Degree in Social Work or related field Preferred
- Experience in case management or healthcare/social services
- Strong communication and coordination skills
