We're hiring a Community Connector to build relationships with referral partners across Fairfield County and connect families with quality care.
You'll spend your days in the field—hospitals, nursing homes, community centers, senior living facilities—meeting with discharge planners, case managers, and care coordinators. When you make a connection that works, seniors get connected with the care they need.
TWO TRACKS
Hungry Hustle: You're early in your career or transitioning. You love outreach, you handle rejection well, you're organized, and you move fast. You track your own work and measure success by relationships built.
Seasoned Operator: You have 5+ years in healthcare business development, service marketing, or sales. You have existing relationships, understand Medicaid, and know how to navigate healthcare systems. You work strategically and bring immediate credibility.
THE JOB
- Field visits to hospitals, SNFs, assisted living communities, senior centers, and Access Agencies across Fairfield County
- Build and maintain referral partnerships
- Follow up on leads and convert to placements
- Represent the agency at community events
- Track outreach activities and feedback
- Meet referral conversion and placement targets
- Weekly check-ins with the CEO
WHAT WE'RE LOOKING FOR
Drive. Be comfortable with phone calls and face-to-face outreach. Be Organized and able to manage your own schedule. Professional demeanor and strong communication skills.
Preferred: experience in development, sales, or healthcare. Experience with Medicaid or senior care. Knowledge of CT healthcare .
COMPENSATION
Part-Time: $20,000 - $55,000 annually (prorated) Full-Time: $45,000 - $80,000 annually
Mileage reimbursement. Performance-based bonuses
