Setting: Community Mental Health / Addiction Services
Location: Salt Lake City, UT
Schedule: Full-time, Monday-Friday, daytime hours

Patient population: adults with serious mental illness, including schizophrenia, bipolar I disorder, major depression with psychotic features, and co-occurring substance use disorders.
Job Description:
- Provide psychiatric evaluations, diagnosis and medication management.
- Treat adults with serious mental illness and co-occurring substance use disorders.
- Provide community-based care through an ACT team.
- Collaborate with an interdisciplinary behavioral health team.
- Monitor treatment, support crisis intervention and coordinate community care.
Requirements:
- Current, unrestricted APRN license in the State of Utah with prescriptive authority
- PMHNP-BC certification through ANCC
- DEA registration
- Current CPR/BLS
- Valid driver's license and insurability; comfortable providing care in a community/field-based environment
- DATA 2000/X-Waiver for buprenorphine prescribing strongly preferred
