Integrated Intake & Counseling Coordinator – Provisional QHP (Tier I) - IP
Full - Time (40 hours per week)
Schedule: M-F, 9:00am to 5:30pm

The Integrated Intake & Counseling Coordinator serves as the primary point of entry for individuals seeking substance use disorder (SUD) and integrated behavioral health services. This position coordinates the intake, assessment, admission, and referral process while ensuring timely access to appropriate levels of care. Working as part of an interdisciplinary team, the Coordinator collaborates with medical providers, clinicians, peer specialists, and community partners to promote engagement, continuity of care, and successful treatment outcomes. Under clinical supervision, the role also provides supportive counseling and care coordination while progressing toward full CASAC credentialing.
Key Responsibilities
- Serve as the primary contact for individuals seeking SUD and behavioral health services.
- Conduct intake screenings, biopsychosocial and substance use assessments, admission evaluations, and LOCADTR 3.0 level-of-care determinations.
- Coordinate admissions, referrals, transfers, and care transitions to appropriate treatment services.
- Collect and verify demographic, insurance, consent, and eligibility information required for admission.
- Educate clients and families about treatment options, program expectations, and recovery supports.
- Provide supportive individual, family, and group counseling under clinical supervision using evidence-based practices.
- Assist in developing individualized treatment plans and coordinate care with medical providers, mental health clinicians, peer staff, and other members of the treatment team.
- Participate in interdisciplinary team meetings and communicate assessment findings and client needs.
- Maintain accurate and timely documentation in the electronic health record in compliance with NYS OASAS, HIPAA, 42 CFR Part 2, and agency standards.
- Coordinate referrals with hospitals, courts, medical providers, and community agencies to ensure continuity of care.
- Assist with client orientation and admission procedures, conduct property and room searches in accordance with agency policy, and help maintain a safe, recovery-oriented environment.
- Participate in supervision, case reviews, quality improvement initiatives, and professional development activities while working toward full CASAC credentialing.
Qualifications
- Bachelor's degree in Psychology, Social Work, Counseling, Nursing (human services specialty), or a related field.
- 1–3 years of experience in health or human services, preferably in substance use treatment or behavioral health.
- Provisional QHP status under NYS OASAS regulations required; actively pursuing CASAC credentialing.
- Valid driver's license required.
Preferred Skills
- Knowledge of substance use disorders, recovery-oriented care, co-occurring mental health conditions, and integrated behavioral healthcare.
- Strong assessment, interviewing, communication, documentation, and organizational skills.
- Experience using electronic health records (EHRs).
- Ability to build positive relationships with clients, families, referral sources, and multidisciplinary team members.
Work Environment
- Primarily office-based clinical setting.
- Occasional lifting of up to 25 pounds.
