Through a virtual-first model, we deliver patient-centered care and support individuals throughout their mental health journey. We are seeking a Director of Case Management to lead and scale our care coordination operations across multiple behavioral health programs.
About the Role
The Director of Case Management will oversee and optimize case management operations to ensure seamless coordination of care across psychiatry, therapy, and outpatient services. This leader will partner closely with physicians, therapists, nurses, and operations teams to drive continuity of care, improve patient engagement, and implement scalable processes that support positive patient outcomes.
Key Responsibilities
- Lead and oversee remote case management operations across multiple behavioral health programs.
- Develop and implement care coordination workflows and patient navigation processes.
- Ensure smooth transitions across levels of care and treatment programs.
- Supervise, coach, and support a distributed team of case managers.
- Collaborate with clinical and operational leaders to align patient care plans and follow-up strategies.
- Monitor patient engagement, compliance, and continuity of care.
- Identify gaps in care coordination and implement process improvements.
- Ensure documentation accuracy, regulatory compliance, and adherence to clinical standards.
- Serve as a liaison between clinical teams and administrative leadership.
Required Qualifications
- Bachelor's degree required; Master's degree preferred.
- 5+ years of experience in case management, care coordination, or clinical operations.
- Experience in behavioral health, psychiatry, telehealth, or integrated care settings.
- Strong understanding of patient navigation and care coordination workflows.
- Proven ability to lead teams and drive operational performance.
- Excellent communication, leadership, and organizational skills.
- Ability to thrive in a fast-paced, high-accountability environment.
Preferred Qualifications
- 2+ years of leadership or supervisory experience.
- Experience managing remote or distributed teams.
- Familiarity with virtual care delivery models and behavioral health operations.
Pay: $90,000.00 - $110,000.00 per year
