Job Title: Home Health RN (Certified Home Care)
Assignment Length: 13+ weeks- extensions are offered!
Location: Charlestown, MA- Newton, MA Area
Shift: Monday through Friday 8a-4:30p. Some weekends
Pay Rate: $73/hr (local under 65 miles) and $75 (travel over 65 miles) +$.76 mileage reimbursement
Job Summary:
As a Home Health RN, responsibilities are not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice.
Also required to perform admission processes and case management responsibilities in accordance with the Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Experience:
- 1 year Certified Home Care experience
- 1 year Case Management experience
Job Requirements:
- RN License in Massachusetts
- Reliable, independent means of transportation to make home visits.
- Fully Vaccinated against Covid-19
