Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using tools like Hoyer lifts and other assistive devices.
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, accurately documenting all patient information in electronic health record (EHR) systems like Epic or Cerner.
- Support medical teams by preparing patients for procedures, assisting with medication administration under supervision, and maintaining aseptic techniques during clinical tasks.
- Provide specialized care for patients with dementia or Alzheimer’s disease, including memory care strategies and behavioral health support.
- Assist in post-acute care settings such as rehabilitation or sub-acute facilities, ensuring smooth transitions and continuity of care for long-term or short-term patients.
- Maintain infection control standards by practicing HIPAA compliance, proper medical documentation, and adhering to safety protocols in high-acuity environments like Level I Trauma Centers or ICU units.
- Collaborate effectively with multidisciplinary teams in outpatient clinics, home & community care programs, or inpatient hospital units to deliver patient-centered service.
Requirements
- Valid CNA certification with current state licensure to practice as a Certified Nursing Assistant.
- Experience working with diverse populations including seniors, children, individuals with developmental disabilities or behavioral health needs.
- Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) certification.
- Prior experience in healthcare environments such as hospitals (including ICU or emergency medicine), nursing homes, assisted living facilities, or hospice care is highly desirable.
- Familiarity with electronic medical records (EMR) systems such as Epic, Cerner, eClinicalWorks or similar platforms for accurate documentation.
- Ability to perform physically demanding tasks including lifting with Hoyer lifts and assisting patients with mobility challenges.
