Upper Bay Counseling & Support Services (UBCSS) is seeking a compassionate and motivated Residential Case Manager to support individuals living with serious and persistent mental illness. This position plays a vital role in helping consumers achieve greater independence, stability, and success in the community through care coordination, advocacy, and recovery-focused services.

What You'll Do
- Coordinate and facilitate access to mental health, medical, social, financial, educational, and community support services
- Develop individualized treatment goals in collaboration with consumers
- Provide recovery-oriented interventions that promote independent community living
- Teach and model independent living skills
- Participate in treatment team meetings and care coordination activities
- Advocate for consumers with healthcare providers, community agencies, and other service organizations
- Support development of social, coping and wellness management skills
- Support clients in developing natural community supports and meaningful social connections
- Initiate face to face rehab support services in the community and residential setting
- Facilitate recovery focused interventions aligned with Individual treatment plans
- Monitor medications and assist with treatment compliance as appropriate
- Maintain accurate and timely documentation within the electronic health record system in accordance with COMAR, Medicaid and agency requirements
- Maintain billable documentation standards and productivity expectations
- Respond to crisis, utilize de-escalation techniques and crisis management strategies
- Transport and accompany consumers to appointments and community resources as needed
- Perform other duties that support consumer recovery and program operations
What We're Looking For
- Bachelor’s degree in social work, Human Services, Psychology, or a related field required
- Experience working with individuals with mental health needs preferred
- Strong written, verbal, and interpersonal communication skills
- Excellent organizational and time-management abilities
- Ability to multitask and prioritize responsibilities in a fast-paced environment
- Strong problem-solving skills and professional judgment
- Computer literacy and experience with electronic documentation systems
- Valid driver's license and good driving record required
