Summary
As a Care Manager at The Arc Greater Hudson Valley, NY, you will play a vital role in supporting individuals with developmental disabilities by coordinating care plans and ensuring they receive the services they need. This position is essential to our mission of providing compassionate and effective support to our clients.
Responsibilities
- Develop and implement individualized care plans for clients based on their unique needs.
- Coordinate with healthcare providers, families, and other stakeholders to ensure comprehensive care.
- Review & Coordinate the medical and dental care of people supported at PVI. Write and maintain medical records /reports.
- Prepare annual /semi-annual reports. Administer immunizations and other IM injections.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Provide guidance and support to clients in accessing community resources and services.
- Conduct regular assessments to evaluate the effectiveness of care plans.
- Supervise direct support staff to ensure quality service delivery.
Requirements
- Experience in patient assessment or case management preferred.
- Strong interpersonal and communication skills for effective collaboration with clients and families.
- Ability to supervise staff and foster a positive team environment.
- Knowledge of developmental disabilities is a plus but not required; training will be provided.
- A passion for helping others and making a difference in their lives.
- Must be licensed LPN or RN
