Position Description: Intake Coordinator - Must speak Spanish/English
Location: York, PA
POSITION SUMMARY: This position performs the initial service intake visit to clients and oversees services provided to individuals in their place of residence who need assistance caring for themselves as a result of disability, age and/or illness.
Requirements:
· Bilingual Fluent in Spanish, Speaking, Reading and Writing
· Must Have Valid DL and Reliable Transportation and Insurance
· Must have a Minimum of 1-year knowledge in Home Health Care
· Interpersonal communication, time management and organizational skills. Evidence of
leadership and/or supervisory activities.
· Knowledge of principles and processes for providing and supervising the delivery of consumer personal care and home management services, including needs identification, meeting quality standards and evaluation of consumers including consumer satisfaction
· Obtains and retains clear criminal records check, FBI, child abuse and required clearances such as OIG, EPLS/SAM and Medicheck.
Responsibilities:
· Completes the client service intake visit to determine needs and direct care worker skill level required in a timely manner meeting care needs, contract service standards and regulatory requirements.
· Develops and modifies, as needed, the assignment/service plan for the direct care worker (DCW).
· Performs periodic (Supervisory, Post Discharge, Monthly, Quarterly & Annually) supervisory visits to client’s homes ensuring service plan and client needs are being met.
· Assists with the basic orientation, training, and competency evaluation of direct care workers.
· Reviews and investigates consumer complaints, tracks & ensures complaint resolution to consumer satisfaction.
· Initiates consumer satisfaction survey based on established schedule.
· Investigate client-based issues such as incidents, events, complaints, and safety concerns with responsibility for initiating reporting and follow-up in coordination with Client Service Director as needed.
· Maintains a professional standard of conduct, projecting a positive image of the
· agency always.
· Complies with infection prevention/control, risk management, and safety policies and procedures.
· Adheres to all policies and practices of the agency to maintain Service Standards.
· Works with consumers and employees in a caring, therapeutic, culturally sensitive professional manner.
· Respects consumer control and the independent living philosophy.
