Overview
We are seeking compassionate and reliable Certified Home Health Aides (CHHAs) to join our team. In this vital role, you will provide personalized assistance to clients, supporting their daily needs and ensuring their safety and well-being. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health through observation of vital signs and overall condition
- Administer medications and perform catheter care following medical instructions and home healthcare regulations
- Support clients with memory care needs such as Alzheimer’s disease and dementia, ensuring a safe environment
- Document patient care accurately using Electronic Health Record (EHR) systems and maintain confidentiality in accordance with HIPAA standards
- Transfer patients safely using patient transfer methods such as Hoyer lifts or other assistive devices
- Provide companionship and emotional support, fostering a trusting relationship with clients
Experience
- Prior experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or end-of-life care
- Assisted living or nursing home experience preferred, with familiarity in home health care assistance and long-term care settings
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic first aid/CPR certifications required
- Experience working with individuals with disabilities or dementia is highly valued
- Ability to operate EMR/EHR systems for accurate medical documentation and client records
- Comfortable performing heavy lifting, cleaning, laundry, cooking, and other household tasks as part of comprehensive home care services
- Proven ability to communicate effectively with clients, families, and healthcare teams while maintaining patient confidentiality
