Job Summary
Join our compassionate healthcare team as a Hospice RN Case Manager, where you will play a vital role in providing holistic, patient-centered care to individuals facing life-limiting illnesses. In this dynamic position, you will coordinate and deliver specialized nursing services in the home or outpatient settings, ensuring comfort, dignity, and quality of life for patients and their families. Your expertise in acute and chronic care, combined with your passion for empathetic patient service, will help guide families through challenging times while promoting seamless care transitions and comprehensive symptom management.
Duties
- Conduct thorough patient assessments to evaluate clinical needs, symptoms, and psychosocial factors impacting end-of-life care
- Develop plan of care and personalized care plans aligned with patient wishes, medical conditions, and hospice protocols
- Provide direct nursing care including medication management, symptom control, and vital signs monitoring
- Collaborate with interdisciplinary teams to coordinate services such as social work and spiritual support
- Educate patients and families on disease progression, medication use, and comfort measures to enhance understanding and peace of mind
- Maintain accurate medical records in compliance with HIPAA regulations and document all patient interactions thoroughly
- Collaborate with the IDG team and hospice Medical Director to coordinate patient care
- Document nursing visits and comprehensive assessments
- Follow hospice Medicare guidelines defined by CMS
Requirements
- Valid RN license in the state of practice with current CPR certification
- Knowledge of medical terminology and physiology are necessary for comprehensive patient assessment
- Strong understanding of hospice philosophy, palliative care principles, and end-of-life symptom management
- Excellent communication skills to effectively educate families and collaborate with healthcare teams
- Ability to manage multiple priorities efficiently within a fast-paced healthcare setting

Pay: $48.00 - $52.00 per hour
