Job Summary
We are seeking a compassionate Palliative Care Nurse Practitioner to join our multidisciplinary healthcare team. This role involves providing comprehensive symptom management, emotional support, and quality-of-life care to patients with serious illnesses. The ideal candidate will have extensive experience in hospice and palliative medicine and a strong background in managing complex medical conditions in a nursing facility setting. Knowledge of electronic health record systems such as PointClickCare is essential to ensure accurate documentation and seamless patient care coordination. Duties
- Conduct thorough patient assessments, physical examinations, and review of medical history using advanced clinical evaluation techniques.
- Develop and implement individualized palliative care plans focusing on symptom control such as pain management.
- Coordinate with multidisciplinary teams for discharge planning, case management, nutrition counseling, and family education.
- Manage complex cases involving hospice & palliative medicine.
- Utilize EMR/EHR systems for accurate medical documentation and coding (ICD-10/CPT).
- Provide updates to family on every visit.
- Ensure infection control protocols are strictly followed during all procedures such as sterile processing or aseptic technique applications.
- Engage in patient education regarding disease processes, medication administration, nutrition, and health coaching strategies.
Skills
- Proven experience in hospice & palliative medicine.
- Expertise in managing complex medical conditions.
- Proficiency with EMR/EHR systems, including PointClickCare, familiarity with medical coding (ICD-10) and CPT coding for billing purposes.
- Strong knowledge of physiology and anatomy relevant to internal medicine, geriatrics, behavioral health issues, and infectious disease protocols.
- Ability to perform diagnostic evaluations using laboratory experience or medical imaging techniques.
- Excellent communication skills for patient education and family support; adept at working with diverse populations.
- Knowledge of HIPAA regulations to ensure patient confidentiality; experience working within NCQA standards is preferred.
