About the Role
The Client Care Coordinator serves as the first point of contact for prospective clients and their families. This position is responsible for coordinating client intake, conducting initial assessments, assisting with service planning, maintaining accurate records, supporting caregiver scheduling, and ensuring every client receives outstanding concierge-level customer service.
Responsibilities
- Conduct client intake interviews and initial assessments.
- Explain company services, policies, and pricing to prospective clients.
- Coordinate admissions and onboarding of new clients.
- Prepare and maintain client files and documentation.
- Obtain signed consents and required forms.
- Assist in developing individualized service plans.
- Schedule client visits and coordinate caregiver assignments.
- Maintain communication with clients, families, caregivers, and referral sources.
- Monitor client satisfaction and follow up on concerns.
- Assist with quality assurance and compliance activities.
- Maintain confidentiality and HIPAA compliance.
- Support marketing and community outreach events as needed.
- Perform other administrative duties assigned by leadership.
Qualifications
- Associate's degree preferred (Bachelor's degree is a plus).
- Minimum of 2 years of healthcare, social services, case management, or customer service experience.
- Experience in home care, hospice, healthcare, or senior services preferred.
- Excellent communication and organizational skills.
- Strong computer skills, including Microsoft Office and electronic documentation systems.
- Ability to multitask while maintaining attention to detail.
- Valid driver's license and reliable transportation.
