We are looking for a motivated, relationship-driven Community Liaison to represent Simplicity Home Care throughout the Denver metro area. This is not a desk job. We are seeking someone who enjoys being in the community, building meaningful relationships, and connecting veterans with the home care services they deserve.
As our Community Liaison, you will educate healthcare professionals, veteran organizations, and community partners about our services while identifying veterans who could benefit from non-medical home care. Your success will come from developing trust, creating long-term partnerships, and becoming a recognized resource within the veteran community.

What You'll Do
- Build strong relationships with hospitals, VA clinics, rehabilitation facilities, skilled nursing facilities, assisted living communities, physicians, social workers, discharge planners, veteran service organizations, senior centers, and other community partners.
- Identify veterans who may benefit from home care services and educate referral sources on how Simplicity Home Care can help.
- Develop and execute a territory outreach plan to increase awareness of our services throughout the Denver market.
- Maintain regular contact with referral partners through in-person visits, phone calls, and follow-up communication.
- Track outreach activities and referral relationships using our CRM.
- Work closely with our operations team to ensure exceptional service for every referral.
- Serve as a trusted ambassador for Simplicity Home Care within the community.

- Organized and comfortable managing a territory.
- Professional appearance and presentation.
- Comfortable speaking with healthcare professionals and community leaders.
- Valid driver's license and reliable transportation.
Preferred Experience
Experience in one or more of the following is strongly preferred:
- Home Care
- Home Health
- Hospice
- Skilled Nursing Facilities (SNFs)
- Rehabilitation Hospitals
- Assisted Living
- Memory Care
- Hospital Case Management
- Medical Sales
- Healthcare Marketing
- Veteran Services
- Senior Living
A bachelor's degree in Business, Marketing, Healthcare Administration, or a related field is helpful but not required.
