Job Summary
We are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure seamless discharge planning and utilization management. Duties
- Conduct comprehensive patient assessments, including intake and social services evaluations, to determine individual needs and develop tailored care plans.
- Coordinate with healthcare providers across hospital departments such as ICU, emergency medicine, pediatrics, and trauma centers to facilitate timely interventions and discharge planning.
- Manage utilization review and utilization management processes to ensure appropriate level of care while adhering to HIPAA regulations and health insurance guidelines.
- Collaborate with social workers, behavioral health specialists, and community resources to support patients’ social determinants of health and facilitate access to necessary services.
- Maintain accurate documentation within EMR/EHR systems, including medical terminology, ICD-9/ICD-10 coding, CPT coding, and discharge summaries.
- Communicate effectively with patients and their families, providing bilingual support when needed, to explain care plans and answer questions clearly.
- Participate in ongoing training on hospital policies, managed care protocols, HIPAA compliance, and medical terminology to stay current with industry standards.
Experience
- Prior experience in hospital settings such as ICU, Level I or Level II trauma centers, or pediatric units is highly preferred.
- Demonstrated knowledge of acute care practices including emergency medicine, hospital medicine, and managed care environments.
- Strong background in case management with familiarity in discharge planning, utilization review, and utilization management processes.
- Proficiency in EMR/EHR systems along with Microsoft Office applications for documentation and communication purposes.
- Experience working with individuals from diverse cultural backgrounds enhances patient engagement and service delivery.
- Knowledge of health insurance policies, medical coding (ICD-9/10), CPT coding, and medical terminology is essential for accurate documentation.
- Bilingual skills are a plus to effectively serve a broader patient population.
