Compensation & Schedule

Key Responsibilities
- Perform in-home physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient needs and clinical goals.
- Provide therapeutic interventions focused on improving mobility, strength, balance, endurance, and functional independence.
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
- Monitor patient progress and modify treatment plans when appropriate.
- Collaborate with Physicians, Nurses, Physical Therapist Assistants (PTAs), and other members of the interdisciplinary healthcare team.
- Complete accurate and timely clinical documentation.
- Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) when appropriate.
- Maintain patient confidentiality and comply with all clinical, ethical, and regulatory standards.
QualificationsRequired
- Active Ohio Physical Therapist (PT) License
- Valid driver's license
- Reliable transportation
- Current automobile insurance
- Ability to work independently in a field-based environment
Preferred
- Home Health experience preferred, but not required
- Experience in Skilled Nursing (SNF), Outpatient, Inpatient, Rehabilitation, or Acute Care
- Familiarity with Medicare Home Health documentation
- Strong communication, organization, and clinical decision-making skills
Work Environment
- Patient care provided directly in patients' homes
- Flexible assignments based on availability and location preferences
- Assignments throughout the Columbus metropolitan area
