Immediately Hiring – PRN Physical Therapists (PT)
Lakehouse Healthcare and Rehabilitation Center is hiring PRN Physical Therapists to join our in-house rehabilitation team. We are seeking therapists with availability to support weekday and/or weekend coverage needs. Availability does not need to include both weekday and weekend shifts; however, all PRN therapists are required to work one Saturday shift per month. This role offers flexibility while providing opportunities to work across a diverse caseload including transitional care and long-term care residents.

What You’ll Do:
- Perform evaluations, develop individualized treatment plans and progress treatment plans based on resident response
- Deliver skilled physical therapy interventions to improve mobility, safety, function, and quality of life
- Collaborate with nursing, therapy, providers, residents, and families to coordinate care
- Supervise and support Physical Therapy Assistants (PTAs) as appropriate
- Complete timely and accurate documentation
Qualifications:
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license and registration
- Skilled nursing experience preferred

Pay: $51.00 - $55.00 per hour
