Case Manager duties include carrying a case load of foster families in and around the Greater Houston area. Case Manager responsibilities include visiting in foster homes twice monthly, assessing child's adjustment to psychological and sociological problems and changes, assessing for services needed and progress towards identified goals. Case Manager will communicate with the foster family by providing supervision, support, training, and documentation. This individual will also monitor foster homes in compliance with State standards and agency policy. Case Manager will serve a liaison with a foster child's school, therapist, psychiatrist, attorney, and other professionals. Case Manager will be responsible for submission of various documentation to include monthly progress reports, intakes reports, discharge reports, individual service plans and other paperwork as needed. This individual will be "on-call" on a regular and equal basis, attend regular staff meetings, and participate in recruitment and fund raising activities. Candidate must have a Bachelor's degree, preferably in social work or related field, be willing to work flexible schedule, and have reliable transportation. This individual must have good verbal and written skills, organization/time management skills, and the ability to meet deadlines. Bi-Lingual English/Spanish preferred.
