Required Qualifications
Minimum requirements include the following:
v A nursing degree from an accredited college or university, or be a graduate of an approved LPN/RN program.
v Two years of experience as an LPN/RN.
v Eligible to participate in federally funded health care programs.
Must also meet state requirements for relevant licensures or certifications and have no disciplinary action in effect against professional license.
At least one year of experience in providing basic restorative services is preferred.
Completed training in restorative nursing services, or complete the training within 90 days of hire or promotion to the position.
Major Duties and Responsibilities
Plans, develops, organizes, implements, and directs restorative nursing services for residents who require those services in the facility.
Assesses residents and makes determinations related to the level of restorative nursing services the residents need. Makes assignments as indicated.
Develops and documents a plan of care that includes measurable objectives and interventions for residents who require restorative nursing services.
Provides oversight of each resident’s restorative nursing program and documents regular (i.e. weekly/monthly) evaluations. Assures daily documentation is recorded by restorative aides. Updates plan of care as indicated.
Performs restorative nursing services in accordance with care plans and facility policies and procedures. These services may include activities related to range of motion, splint/brace assistance, bed mobility, transfers/walking, dressing/grooming, eating/swallowing, amputation/ prosthesis care, and communication.
Maintains current lists of residents on restorative caseload and residents who require splints, restorative dining, or focused weight monitoring.
Supervises dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
Oversees the weighing of residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse manager and physician.
Tracks the condition of splints/braces of residents who require them. Reports lost or broken splints/braces to nurse manager, therapist, and/or physician as indicated.
Reports changes in a resident’s condition to the resident’s nurse manager and physician.
