Job Summary
We are seeking a dedicated and highly skilled Registered Nurse Care Manager to coordinate comprehensive patient care within our healthcare organization. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine experience, along with a strong understanding of health regulation policies and patient-centered care principles. As a Registered Nurse Care Manager, you will serve as a vital link between patients, families, and multidisciplinary healthcare teams to ensure optimal health outcomes, efficient utilization of resources, and seamless discharge planning.
Responsibilities
- Conduct thorough patient assessments utilizing vital signs, physiological knowledge, and medical terminology to develop individualized care plans.
- Manage inpatient and outpatient caseloads across diverse settings such as hospital medicine, emergency rooms, intensive care units (ICU), pediatric units (PICU), and post-acute care environments.
- Coordinate diagnostic evaluations, manage medical records, and oversee electronic health records (EHR) management to ensure accurate documentation and compliance with HIPAA regulations.
- Lead utilization management efforts by evaluating medical necessity, facilitating appropriate resource allocation, and supporting managed care initiatives.
- Collaborate with healthcare providers to develop discharge plans that incorporate post-acute care options and community resources for smooth patient transitions.
- Perform patient service activities including intake assessments, managing patients as a nurse case manager, and ensuring high-quality patient care aligned with hospital policies.
- Apply knowledge of infectious disease care, physiology, and hospital experience to address complex clinical situations effectively.
- Participate in quality improvement initiatives focused on patient safety, health regulation policy adherence, and clinical best practices.
Skills
- Extensive critical care experience with proficiency in ICU, trauma center (Level I & II), emergency medicine, and surgical environments such as the operating room (OR).
- Strong background in hospital medicine, medical-surgical nursing, geriatrics, behavioral health, pediatrics (including PICU), and outpatient clinic settings.
- In-depth understanding of diagnostic evaluation procedures, inpatient/outpatient workflows, and post-acute care processes.
- Knowledge of HIPAA regulations, medical terminology, vital signs monitoring, and physiology fundamentals essential for comprehensive patient assessment.
- Proven ability to manage complex cases through effective case management strategies and utilization management practices.
- Familiarity with health regulation policies governing hospital operations and patient safety standards.
- Excellent communication skills for engaging with multidisciplinary teams, patients, families, and external providers while maintaining a focus on patient service excellence.
- Competence in electronic health records (EHR) systems management to support accurate documentation and regulatory compliance.
