About the Role
As a Licensed Practical Nurse (LPN) in Home Health, you'll provide high-quality nursing care to patients in their homes under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team. You'll play an essential role in helping patients manage their health, maintain independence, and improve their quality of life.

What You'll Do
- Provide skilled nursing care according to individualized care plans.
- Administer medications and treatments safely and accurately.
- Monitor patient conditions and report changes to the supervising RN.
- Collaborate with physicians, case managers, caregivers, and healthcare team members.
- Educate patients and families on disease management, medications, and treatment plans.
- Maintain accurate and timely clinical documentation.
- Support patients with dignity, compassion, and respect.
- Participate in care conferences, team meetings, and ongoing training.
What We're Looking For
Required Qualifications
- Graduate of an accredited Practical Nursing program.
- Current, unrestricted Massachusetts LPN license.
- Successful completion of NCLEX-PN.
- Current CPR certification.
- Strong clinical assessment, communication, and documentation skills.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to meet required health screening and clearance requirements.
- Professional references demonstrating clinical competence and reliability.
Preferred Qualifications
- Associate degree.
- Two or more years of nursing experience.
- Experience in home health, hospice, long-term care, community health, or related settings.
