Social Worker Position
Solano Adult Day Health Care Center (ADHC) is a Community Based Adult Services Program in stable business for over 19 years, dedicated to serving Solano County Adult (18+) residents with medical or mental health or Dementia, Alzheimer’s diagnosed conditions. Solano ADHC is a reputable, well managed and stable organization, licensed by Department of Public Health.
We are seeking a dedicated Social Worker with a Bachelor’s or Master’s Degree in Social Work or Psychology or Psychology Counselling or Gerontology.
You will be a part of the Multidisciplinary team working with other Staff members to provide the best care for our program participants.
Our CBAS services include Nursing, Physical Therapy, Occupational Therapy, Social Services, Psychosocial counseling, Dietetics, Therapeutic Activities etc.,
JOB RESPONSIBILITIES:
· Conduct or Assist with Social Services Initial Assessment for program eligibility and Semi-annual reassessments to develop and implement individual Social Services Care Plan.
. Chart and document social services offered.
· Be responsible for In-Take services which includes orienting visitors, family members and new potential participants to the program. Assist with enrollment process.
· Receive Psychosocial consultation from LCSW and assist LCSW as needed.
· Prepare quarterly progress reports and other reports as necessary.
QUALIFICATIONS:
The Social Worker Candidate will require:
At minimum a bachelor’s degree or master’s degree in social work, Psychology, Psychology Counselling or Gerontology. New College Grads with or without experience are welcome to apply. We have both entry level and Senior positions and also both P/T and F/T positions available.
Compensation will be based on education and experience. Training will be provided.
Computer Skills: Proficient- MS Word is a must and EXCEL is a plus.
Good oral and written communication skills and effective time management skills a plus
Work Schedule: Monday – Friday 9:30 AM to 5:00 PM (Flex hours for P/T position possible)
Compensation: Job Type: Full-time (35-40 hours/week) OR Part-Time (15-20 hours/week).
Pay for F/T positions: $48,000.00 - $52,000.00 per year
Pay for Part-Time positions: $24/hour to $26/hour based on education and experience.
