Exodus Recovery, Inc. (Exodus) is looking to hire a Crisis Transition Coordinator for our Crisis Transition program.
Position Summary:
Provide intensive case management/navigation for up to 60 days following discharge for clients in the Exodus Recovery Urgent Care Centers. Interventions will be targeted to ensure continued crisis stabilization, linkage to ongoing services and supports and triage services within the individual’s local community. The Crisis Transition Team Navigator shall meet the qualifications as indicated in the Short-Doyle Medi-Cal Rehab Option Manual and provide services in accordance with the scope of their practice.
MINIMUM QUALIFICATIONS:
- Master’s degree in health sciences related field required ( e.g Social Work , Psychology , Counseling , or related behavioral health discipline) with three (3) years clinical experience.
- Excellent knowledge of therapeutic interventions, assessment, community resources and crisis intervention skills
- Ability to work independently and exercise sound clinical judgement.
- Excellent verbal and written communication and interpersonal skills.
- Ability to manage multiple priorities in a fast-paced enviroment.
- Valid California driver's license with acceptable driving record with ability to travel to community-based locations as needed.
- Spanish preferred but not required.

Pay: $70,720.00 - $72,800.00 per year
