Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the appropriate services and support throughout their healthcare journey. You will work closely with patients, families, and healthcare providers to develop individualized care plans that promote optimal health outcomes. This position requires a strong background in nursing, excellent communication skills, and a commitment to patient service.
Responsibilities
- Assess and evaluate patients' needs to develop comprehensive care plans tailored to individual circumstances.
- Coordinate with multidisciplinary teams to ensure continuity of care across various settings, including acute care and managed care environments.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Educate patients and their families about health management, treatment options, and available resources.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Advocate for patients by facilitating access to necessary services and resources within the healthcare system.
- Document all patient interactions and updates in accordance with organizational policies and standards.
- Collaborate with healthcare providers in Level I Trauma Centers, PICU, ICU, pediatrics, geriatrics, and other specialties as needed.
Requirements
- Current Registered Nurse (RN) license in OK.
- Previous experience in case management or a related nursing field is preferred, not required.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Familiarity with HIPAA regulations and commitment to maintaining patient confidentiality.
- Ability to work independently while managing multiple cases effectively.
- Strong organizational skills and attention to detail in documentation and reporting.
