Position Summary
Nova Home Care Agency LLC is seeking a compassionate, dependable, and highly organized Licensed Practical Nurse (LPN) on a PRN basis to support our clients, caregivers, and clinical operations.
The LPN will work under appropriate RN/agency supervision and within the LPN's authorized scope of practice. Responsibilities may include client visits, observing and documenting client status, supporting care-plan implementation, communicating changes in condition, caregiver education and support, documentation review, and assisting the agency's clinical team as needed.
This is an excellent opportunity for an LPN who is looking for a flexible PRN position and enjoys working with seniors, individuals with disabilities, families, and caregivers in the home care setting.
Essential ResponsibilitiesClient Visits & Monitoring
- Conduct assigned client visits in accordance with agency policies and the established plan of care.
- Observe and document the client's general condition, functional abilities, safety concerns, and changes in status.
- Obtain and document vital signs when appropriate and authorized.
- Observe clients for changes that may require additional assessment or intervention by the supervising RN or other appropriate healthcare professional.
- Promptly report significant changes in a client's condition to the supervising RN and/or Administrator according to agency procedures.
- Assist with follow-up visits as assigned.
- Reinforce established care and safety instructions with clients, families, and caregivers.
- Identify potential home safety concerns and communicate them to the appropriate agency representative.
Care Plan Support
- Review assigned client care plans and ensure that caregiver services are consistent with the established plan.
- Assist the RN with implementing and monitoring care plans as delegated and permitted.
- Report concerns regarding whether a client's current needs are being adequately addressed.
- Communicate recommendations or observations to the supervising RN for further evaluation.
- Help ensure that changes approved by the appropriate clinical professional are communicated to caregivers.
Caregiver Support & Supervision
- Observe caregivers during client visits when assigned.
- Help ensure caregivers understand and follow the client's established plan of care.
- Provide education, instruction, and reinforcement to caregivers within the LPN's scope and agency policies.
- Identify caregiver performance or training concerns and report them to the RN or Administrator.
- Assist with caregiver orientation and skills education as requested.
- Promote proper infection prevention, client safety, fall prevention, and emergency procedures.
Documentation & Quality Assurance
- Complete accurate, timely, and professional documentation for all assigned client visits.
- Review assigned caregiver documentation for completeness and report concerns or inconsistencies.
- Assist with clinical record reviews and internal quality-assurance activities.
- Help identify missing, incomplete, or inconsistent documentation.
- Maintain confidentiality of client and employee information.
- Follow HIPAA requirements and agency privacy and security policies.
- Participate in agency audits, quality-improvement activities, and corrective-action processes when requested.
Communication & Coordination
- Maintain professional communication with clients, families, caregivers, the supervising RN, Administrator, and other members of the care team.
- Communicate client concerns promptly and accurately.
- Participate in care coordination discussions when requested.
- Document relevant communications according to agency procedures.
- Escalate emergencies, significant changes in condition, suspected abuse or neglect, medication concerns, and other reportable issues according to agency policy and applicable requirements.
Agency & Administrative Support
When needed, the PRN LPN may also assist with:
- Reviewing client records for completeness.
- Preparing documentation for RN review.
- Supporting new-client onboarding.
- Assisting with caregiver orientation and education.
- Reviewing incident documentation.
- Participating in staff meetings or clinical meetings.
- Supporting infection-control and safety initiatives.
- Helping maintain organized and compliant clinical records.
- Identifying opportunities to improve agency documentation and care processes.
Qualifications
Required:
- Current and unrestricted Licensed Practical Nurse (LPN) license in Virginia or other authorization permitting practice in Virginia.
- Graduate of an approved practical nursing program.
- Current CPR/BLS certification as required by the agency.
- Ability to successfully complete required background and employment screenings.
- Strong communication and documentation skills.
- Ability to work independently while following appropriate RN supervision and agency policies.
- Reliable transportation for client visits.
- Valid driver's license and automobile insurance when driving is required for the position.
- Ability to maintain client confidentiality and comply with HIPAA requirements.
Preferred Qualifications
- Previous home care or home health experience.
- Experience working with seniors and individuals with disabilities.
- Experience with dementia or Alzheimer's care.
- Experience with chronic medical conditions.
- Experience with caregiver education or supervision.
- Familiarity with Medicaid or managed-care home care programs.
- Experience with electronic health records, EVV, or home care documentation.
- At least 1 year of nursing experience preferred.

Schedule
This is a PRN / as-needed position.
Hours may vary depending on:

PRN status does not guarantee a specific number of hours per week.
Some local travel throughout Northern Virginia may be required.

Compensation
