CLIENT CARE MANAGER (CCM) / SCHEDULER, DELRAY BEACH

Job Summary: As the Client Care Manager, you will be responsible for managing the client and employee scheduling, engagement, and interaction with all aspects of inquiries and conversion to client, overseeing, coaching, and managing the caregivers/ employees for all private-duty and skilled home care clients and staffing partners. Main Responsibilities
- Customer/client care – effectively manages the customer care procedures and serves as point of contact for all new and ongoing clients/customers/staffing relations.
- Will serve as main point of contact for caregivers, employees, and staff.
- Scheduling, care coverage, and coordination – supports and uses all technology tools and resources to ensure proper business processes; schedules staff to meet client care needs and minimize non-billed overtime; ensures on-call phone and assigned staff are properly trained and available as needed.
- CCM is responsible for working with home health aides (HHA’s) and certified nursing assistants (CNA’s), Licensed Practical Nurses (LPN’s), and Registered Nurses (RN’s)
- Problem solves; addresses issues raised by staff, employees / or caregivers, and clients.
- Billing and coding management.
- Assists with the intake of new clients and informs potential clients of our services and reasons to utilize BrightStar over other agencies.
- Responsible for inquiry calls and conversion, follow-up’s and engages with potential clients and family members.
- Assists in the development of organization goals and agency performance improvement activities.
- Responsible for meeting metrics and Key Performance Indicators (KPIs).
- Assists with onboarding of new employees and orientation.
- Conducts ongoing credentialing, and ongoing performance evaluations; to ensure we are maintaining compliance.
- Administers agency policies and procedures.
- Assists the Director of Nursing with assessing the competency of staff to promote quality, continuity, and safety of a client’s care.
- Supervises, educates, and coaches other field personnel, as directed by the Director of Nursing.
- Provide information to be used for statistics, reports, and records for purposes of evaluation and reporting of organization activities.
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
- Assures compliance with all local, state, and federal laws.
- Rotating On-Call as/ when needed.
- Other tasks as needed as requested and required by office administration.
Requirements
- High School Diploma required.
- Certified Nursing Assistant, Home Health Aide, or LPN is a plus, but not required.
- Flexibility and good time management skills is A MUST.
- Must be a self-starter and able to work in a high-volume office environment.
- Must be goal oriented.
- Must be able to work independently but also be a TEAM player.
- Must be empathetic, positive, engaging, outgoing, and friendly.
- Minimum of 2 years’ experience as Customer Care Manager and/or managing customer service within a healthcare office setting; knowledge of HIPAA and healthcare office regulations.
- Excellent organization, planning, and project management skills.
- Valid driver’s license, vehicle, and auto insurance.

Pay: $41,600.00 - $43,680.00 per year
