We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team in New Jersey. You will play a critical role in helping patients maintain their independence and improve their overall well-being.
Responsibilities
-Conduct intake admissions and follow up visits as needed or required
- Ensure compliance with documentation guidelines
- Coordinate care with other nurses on case and management as needed
- Adhere to infection control protocols to maintain a safe environment for patients
-conduct audits, education, training, and other tasks when in office
-Conduct updates on our EHR system Alora plus
-Assist with other administrative or clinical tasks as needed
Qualifications:
- Valid RN license in the state of New Jersey or Compact state license
- Minimum of 2 years of nursing experience, preferably in home health or a related field, Pediatric experience a plus.
- Proficient in using electronic medical record systems
- Strong communication and interpersonal skills
- Ability to work independently and prioritize tasks effectively
