Job Title: Youth Experiencing Success (YES) Career Planner
Reports to: Site Director
Position Description:
This is an active, intensive case management position with employment, post-secondary and training placement that requires a willingness to engage and build relationships with out-of-school youth and their families. Program coordination and intervention services are directed towards student re-entry and engagement in an academic environment. Knowledge of issues pertaining to high-risk youth is preferred, as is knowledge of city resources.
Responsibilities and Duties:
Recruitment, Intake and Assessment
- Enroll prospective out of school youth for ASN/WIOA program with documentation supporting WIOA eligibility
- Work collaboratively with student to create full IWDS 2.0 profile including ISS Plan with each student to create the Individual Service Strategy in IWDS 2.0 which will outline student programming to include goal development (employment or post-secondary), with supportive services and activities which are documented in case notes, all of which must be updated a minimum of every 30 days
- Maintain organized, accurate, up-to-date physical student files to document academic progress, assessment results, documentation of meetings, conferences, etc.
- Maintain organized online IWDS 2.0 student profiles with updated required documents
- Submit monthly reports to ASN Program Service attendance, progress
- Attend follow-up meetings
- Communicate with appropriate training staff to monitor all progress
- Creative and practical problem-solving abilities are assets to the role.
Case Management for ASN students who have enrolled in school/training programs
- Monitor participants engagement and referral follow-up Bi-weekly
- Conduct ongoing communication with participants
- Engage and build relationships with students through career counseling
- Document participants and collateral contacts, services, progress, and barriers in case notes regarding training progress
- Conduct in-person follow-up with participates next steps to facilitate post-secondary education, training and/or employment
- Work with participants to continue developing their work preparedness and skill building to ensure a proper matchup with contractors after completion of training
- Work with participants to continue to develop their work preparedness skills of punctuality, consistent attendance, working well with others/teamwork, task completion, communication skills with others including the participants’ immediate work experience supervisor and other work preparedness skills.
- Continue career assessment with participants to help determine occupational skills and interests.
- Case management includes ;individual follow-up with participation throughout length of program.

Expectations of Career Planner
Preferred Qualifications:
- ·Bachelor’s degree preferred not required or equivalent work experience
- ·Broad knowledge and experience in workforce development, youth work, youth development
- Above average skills in data collection, analysis, and reporting.
· General knowledge of: barriers, challenges and larger systems in the lives of urban youth/families; interventions that promote and support client success. Ability to work cross-culturally
· Proficient with the Internet and Microsoft Office -particularly Word, Excel, and PowerPoint
- · Requires use of a car with valid driver’s license and current insurance
Pay: $48,000.00 per year
