SUMMARY:
ODMF is launching a new Permanent Supportive Housing (PSH) program in Pierce County and is seeking a dedicated Case Manager to support families experiencing chronic homelessness. This role provides trauma-informed, culturally responsive, and Housing First-oriented services to help families achieve long-term housing stability and improved well-being. Services are voluntary, flexible, and delivered in participants’ homes and community settings. The Case Manager will work closely with participants and community partners to reduce barriers, promote stability, and ensure access to critical resources.
LOCATION: Hybrid in Pierce County, WA (2-3 days in office, remainder remote)
RESPONSIBILITIES:
Provide Direct Services
- Deliver intensive, trauma-informed case management using Housing First, harm reduction, and culturally responsive approaches
- Co-create individualized service plans (ISPs) focused on housing and economic stability
- Support participants in achieving their goals through ongoing engagement and follow-up
Support Housing Stability
- Provide ongoing case management support to tenants already placed in Permanent Supportive Housing (PSH) units
- Build and maintain strong partnerships with community organizations to enhance access to resources and services
- Work collaboratively with property management companies to support tenant success and address housing-related needs
- Support housing retention by proactively addressing barriers and promoting stability among residents
- Provide crisis prevention and intervention planning to help maintain long-term housing stability
Promote Economic Stability
- Assist with benefits navigation and applications
- Coordinate financial assistance and resources for housing stability
Coordinate Care & Services
- Connect participants to health and behavioral health services
- Provide or link to independent living skills training
- Collaborate with family members, providers, and community partners
Engage Participants & Community
- Facilitate tenant engagement and community-building activities
- Provide transportation support (e.g., ORCA cards, mileage coordination for home visits)
- Build strong, trusting relationships with participants
Documentation & Reporting
- Maintain accurate and complete participant records
- Enter data timely and accurately into HMIS and other systems
- Ensure compliance with program and funder requirements
Additional Duties
- Actively integrate advocacy into all aspects of work by promoting equity, inclusion, and systems change in support of individuals with intellectual and/or developmental disabilities and their families. This includes ensuring that advocacy efforts are woven throughout program implementation, outreach activities, and team collaboration.
- Other duties as assigned by supervisor.
SKILLS & QUALIFICATIONS:
- Bachelor’s degree in social work, public administration, housing, or a related field (An equivalent combination of experience and education will be considered).
- Experience in housing services, case management, or equity-focused work preferred.
- Knowledge of housing programs, HUD regulations, and fair housing laws.
- Experience with HMIS data collection, reporting, and program monitoring.
- Familiarity with Pierce County housing systems and community resources is preferred.
- Strong communication and interpersonal skills.
- Demonstrated cultural competency and ability to build trust with diverse communities.
- Strong problem-solving skills and ability to navigate complex systems.
- Must be mission-driven, trustworthy, and diligent.
- Must be comfortable navigating technology and online platforms such as Microsoft Office, Salesforce, etc.
- Strong personal commitment to equity and progressive social, racial, and economic justice for people with disabilities.
- Strong verbal and written English communication skills; second language proficiency a plus
- Strong organizational and time management skills.
- Proven ability as an effective communicator and strong advocate for people with disabilities and their families.
- Track record of respect for others’ experiences, opinions, language, values, culture, and knowledge.
- Demonstrated commitment and ability to collaborate while working in a highly diverse environment.
- Ability to pass DSHS criminal background check upon job offer & Every 2 Years.

WORK ENVIRONMENT
- Based in Pierce County with a combination of 3 days in office, community-based, and remote work.
- Requires collaboration with clients, landlords, community partners, property management companies and government agencies.
- Includes in-person outreach, meetings, and cross-system coordination.
PHYSICAL REQUIREMENTS
- Ability to lift up to 40 lbs.
