Registered Nurse – Home Health & Hospice

Position Overview
We are seeking a dependable and compassionate Registered Nurse (RN) to join our growing home health and hospice team.
This position is structured as a dual home health and hospice role. Initially, approximately 90% of the position will consist of home health nursing and 10% hospice nursing. As our hospice census grows, the percentage of hospice responsibilities will increase and the home health portion will decrease accordingly.
The ideal candidate is comfortable working independently, managing their own patient schedule, traveling throughout the greater Houston area, and providing high-quality clinical care in patients' homes.
Primary Responsibilities
Home Health Nursing
- Provide skilled nursing services to home health patients in accordance with physician orders, the plan of care, and applicable regulatory requirements.
- Complete comprehensive assessments, follow-up visits, recertifications, resumptions of care, and other required skilled nursing visits.
- Manage patients with complex medical conditions and identify changes in condition requiring intervention or physician notification.
- Coordinate care with physicians, therapists, aides, patients, families, and other members of the interdisciplinary team.
- Maintain accurate, timely, and complete clinical documentation in the agency's EMR.
- Meet established weekly productivity/visit point requirements.
- Manage an assigned caseload while maintaining appropriate scheduling and timely completion of required documentation.
- Educate patients and caregivers regarding medications, disease processes, treatment plans, safety, and appropriate use of healthcare resources.
- Participate in care coordination and case conferences as required.
Hospice Nursing
As the agency's primary hospice RN, this position will also be responsible for providing nursing services to our hospice patients and supporting the growth of our hospice program.
Responsibilities include:
- Provide skilled hospice nursing care consistent with the patient's plan of care and hospice philosophy.
- Perform hospice admissions, routine visits, symptom management visits, and other required hospice nursing services.
- Assess and manage pain, symptoms, and changes in patient condition.
- Communicate with physicians, hospice leadership, patients, and families regarding changes in condition and plan of care.
- Coordinate hospice services with the interdisciplinary team.
- Complete required hospice documentation accurately and within established timeframes.
- Provide education and support to patients and caregivers regarding end-of-life care.
- Respond to urgent hospice needs and provide appropriate clinical intervention.
- Pronounce hospice patients upon death in accordance with applicable law, regulations, and agency policy.
- Remain available around the clock for hospice patient needs and death pronouncements, including after-hours, weekends, and holidays, as required.
- Travel to extended service areas when necessary to provide hospice services.
Travel & Service Area
Because hospice patients cannot always be concentrated in a single geographic area, this position requires a willingness to travel throughout the greater Houston area and surrounding cities.
Our agency does not decline hospice admissions solely because of a patient's location when the patient is within our greater Houston service area. Candidates must therefore be comfortable with occasional extended travel to meet the needs of our hospice patients.
Qualifications
- Current, unrestricted Texas Registered Nurse (RN) license.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and appropriate automobile insurance.
- Previous home health nursing experience.
- Hospice experience required for the right candidate.
- Strong clinical assessment and critical-thinking skills.
- Ability to work independently and manage a flexible field-based schedule.
- Strong communication and documentation skills.
- Ability and willingness to provide hospice on-call coverage and pronounce patients when required.
- Ability to travel throughout the greater Houston area and surrounding communities.

Pay: $90,000.00 - $100,000.00 per year
