Position Summary
We are seeking a skilled and compassionate Registered Nurse (RN) to join a Long-Term Acute Care (LTAC) team. In this role, you will provide care to medically complex, high-acuity patients requiring extended hospitalization and advanced clinical support.
Responsibilities
Provide direct patient care for chronic and critically ill patients in an LTAC setting
Manage ventilators, tracheostomies, and respiratory therapies in collaboration with respiratory therapy
Administer IV medications, antibiotics, and continuous drips (cardiac, sedation, insulin, etc.)
Perform and document comprehensive patient assessments and monitor for changes in condition
Deliver advanced wound care, including wound vacs and pressure injury management
Monitor cardiac rhythms (telemetry) and respond to acute events
Coordinate care with interdisciplinary teams including physicians, RT, PT/OT, and case management
Educate patients and families on treatment plans and discharge goals
Patient Population
Ventilator-dependent and tracheostomy patients
Post-ICU recovery patients
Complex infections requiring long-term IV therapy
Neurological conditions (stroke, TBI, spinal cord injuries)
Multi-system chronic illnesses
Requirements
Active RN license (state-specific or compact)
BLS , ACLS
Minimum 1 year of RN experience (LTAC, ICU, PCU, or Med-Surg preferred)
Experience with ventilators, trachs, or high-acuity patients
Strong clinical judgment and critical thinking skills
