Job Title: Licensed Practical Nurse (LPN)
Location: Yuma, AZ (85365)
Shift: Day Shift – 12-hour shifts
Schedule: 36–48 hours per week (alternating weeks)
Shift Timing: 6:00 AM – 6:00 PM / 6:30 AM – 6:00 PM
Pay Rate: $40 – $42 per hour
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality care to geriatric residents in a long-term care setting. This role involves delivering direct patient care, administering medications, performing treatments, and supporting the interdisciplinary healthcare team to ensure a safe and effective care environment.
The LPN may also take on leadership responsibilities, including supervising Certified Nursing Assistants (CNAs) and assisting with unit operations under the direction of a Registered Nurse (RN) or physician.
- Administer medications in accordance with physician orders and scope of practice
- Perform clinical treatments such as wound care, catheter care, and other nursing procedures
- Monitor, assess, and report changes in patient condition and response to treatment
- Assist in the development and implementation of individualized care plans
- Conduct admission assessments and maintain accurate and timely documentation
- Provide direct patient care, including assistance with Activities of Daily Living (ADLs)
- Communicate effectively with patients, families, and interdisciplinary care teams
- Participate in shift reports and care coordination activities
- Ensure availability and proper use of medical supplies and equipment
- Supervise and support CNAs and other support staff as needed
- Participate in infection control and quality improvement initiatives
- Attend staff meetings, training sessions, and continuing education programs
- Active Licensed Practical Nurse (LPN) license (State of Arizona or compact license)
- Experience in geriatric or long-term care preferred
- Strong clinical, organizational, and patient care skills
- Excellent communication and teamwork abilities
- Ability to work effectively in a fast-paced healthcare environment
