Job Description
We are seeking an experienced Hospice Registered Nurse to provide compassionate, in-home care to patients throughout the Racine and surrounding service area.
Schedule: Saturday & Sunday | 8:00 AM – 8:00 PM
Contract: 10 weeks | 24 hours per week
Responsibilities
- Provide skilled nursing care and focused patient assessments in patients’ homes
- Respond to urgent hospice needs, including symptom management and post-fall or death visits
- Perform wound care, Foley catheter care, feeding tube troubleshooting, and medication management
- Communicate patient updates and collaborate with the hospice care team
- Document care accurately using the EMR
Requirements
- Active RN license in Wisconsin
- Minimum 2 years of Home Health or Hospice experience
- BLS certification
- Valid driver’s license, reliable vehicle, and auto insurance
- Comfortable working independently in a home-based hospice setting
