Summary of Position:
The Service Coordinator is responsible for supporting the successful implementation of Service Coordination. This position follows the work from home policy of the Family and Children First Council.
Key Responsibilities:
- Plan, develop and implement Service Coordination
- Guide families who are newly referred to Service Coordination through the intake process
- Attend weekly staff meetings as requested.
- Report regularly to the Executive Director on plans, progress, and performance of program.
- Maintain awareness and knowledge of family systems, contemporary youth development and prevention theories, and methods. Ensure activities meet with and integrate grant requirements for quality management, health and safety, legal stipulations, school policies and general duty of care
- Emphasize fidelity to strength based, culturally sensitive assessment and planning process
- Monitor customer satisfaction with the Service Coordination process, the plan, and the services that result from developed plans in conjunction with the evaluation plan.
- Develop public awareness strategies to inform the community about FCFC
- Be sensitive to parent support issues relative to the Service Coordination process and work closely with community partners to support the family
- Provide representation at other meetings, including organizational planning sessions related to the work of the Service Coordination process
- Become certified in completing Child and Adolescent Needs and Strengths (CANS) Assessments and complete regular CANS assessments for youth as assigned
- Provide assistance to any requests for information or questions pertaining to the Service Coordination processes and continue to develop the role as a community liaison with federal/state/county agencies and parent organizations
- Work with systems and providers to develop services required to meet needs of children and families in the county
- Assist in the development and maintenance of tracking system(s) for information including OASCIS
- Administer the Child and Adolescent Needs and Strengths (CANS) Assessment to each child/youth enrolled in Wraparound within two weeks of enrollment in Service Coordination and every 90 days while the child/youth is enrolled.
- Engage in regular trainings as approved by FCFC Director in but not limited to Trauma-Responsive Care, Service Coordination, and other related topics
- Provide weekly reports as requested by FCFC Director
- Other duties as assigned
Minimum Qualifications:
- Has a High School degree and a minimum of two years’ experience, or the equivalent in experience and training in wraparound and service coordination.
- Successful experience working with children and families from a strength-based perspective.
- Has extensive knowledge of group dynamics and family centered system of care practices
- Has training in cultural competency and group facilitation as well as experience in strength-based assessments
- Has experience in training small/large groups
- Has knowledge and experience employing conflict resolution and problem-solving strategies
- Has good written and oral communication skills
- Has strong organizational skills
- Has extensive knowledge of the services that are available in Erie County
- Is experienced in using Zoom and Microsoft Office
- Has reliable transportation, a valid Ohio driver’s license and proof of car insurance
- Is willing to work flexible hours with some evening and weekend work
Physical Requirements:
- Alternate frequently between sitting/standing/walking throughout an eight (8) hour workday.
- Lift/carry a minimum of eight (8) pounds.
- Reaching with bilateral upper extremities into/up to/down to various heights.
- Push/pull with twenty (20) pounds of initial force.
- Climb onto stepladder (2 steps).
- Bend/squat/crouch/kneel.
- Able to ascent/descend stairs.
