We are seeking a Transition Specialist to support individuals with intellectual and developmental disabilities as they prepare for meaningful adult lives through skill development, career exploration, employment readiness, community engagement, and individualized support.

- Teach workplace readiness, self-advocacy, independent living, and social skills through hands-on learning experiences.
- Collaborate with families, schools, employers, OVR counselors, Support Coordinators, and community partners to support successful transition outcomes.
- Complete required documentation and support individuals in pursuing competitive integrated employment opportunities.

What We’re Looking For
- High School Diploma or equivalent with a minimum of 2 years of experience working with individuals with intellectual and developmental disabilities (I/DD)
- Experience working with individuals with intellectual and developmental disabilities.
- Experience with transition-aged youth, employment services, education, workforce development, or community integration.
- Strong communication, organization, and relationship-building skills.
- Ability to work independently and support individuals in achieving meaningful goals.
- Valid driver's license.
- Required clearances and background checks.
- ACRE and/or CESP certification preferred or willingness to obtain.

Pay: $17.10 - $20.90 per hour
