Helen Porter Rehabilitation & Nursing, part of University of Vermont Health and Porter Medical Center, is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care Unit on the evening shift.

Our Memory Care Unit provides specialized care to residents living with Alzheimer's disease and other forms of dementia, focusing on safety, dignity, engagement, and quality of life.

As an RN or LPN on our Memory Care Unit, you will:

Provide skilled nursing care to residents with Alzheimer's disease and other memory-related conditions

Assess, monitor, and document resident health status
-
Administer medications and treatments according to physician orders

Respond appropriately to changes in resident condition

Collaborate with nursing assistants, providers, therapists, and interdisciplinary team members
-
Promote resident safety, comfort, and emotional well-being
-
Support residents with dignity, patience, and compassion

Maintain accurate clinical documentation and communication

Help create a positive, structured environment that supports residents' unique needs

Current Vermont Registered Nurse (RN) license or eligibility for Vermont licensure

Current Vermont Licensed Practical Nurse (LPN) license or eligibility for Vermont licensure

Experience in memory care, dementia care, long-term care, skilled nursing, rehabilitation, assisted living, hospital, or acute care settings

Strong clinical assessment and critical thinking skills

Excellent communication and interpersonal abilities
