Job Title: Advanced Practice Provider – Heart Failure
Location: Lebanon, PA

What You’ll Champion:
-
M-F 8 AM – 5 PM schedule. No call responsibility
-
Rotating outpatient/inpatient schedule for variety and skill development
-
Adult-only cardiology patients with a focus on heart failure management
-
Provide comprehensive care for heart failure patients including diagnosis, medication management, treatment planning, and patient education
-
Serve as a primary point of contact for patients with chronic cardiovascular conditions

Qualifications
Minimum Education:

Masters Degree Nurse Practitioner Required or
-
Bachelors Degree Physician Assistant Required
-
Masters Degree Physician Assistant Preferred
Work Experience:
Licenses:

Certified Registered Nurse Practitioner Upon Hire Required or
-
Licensed Physician Assistant Upon Hire Required and
-
Basic Life Support Upon Hire Required or
-
Advanced Cardiac Life Support Upon Hire Required
Courses and Training:

BLS and/or ACLS as appropriate for the clinical role. Upon Hire Required and

Nurse Practitioner - board certified by American Nurses Credentialing Center Commission on Certification Upon Hire Required and

Nurse Practitioner - Completion of an accredited program for nurse practitioners Upon Hire Required or

Physician Assistant - Graduate of accredited physician assistant program Upon Hire Required
