A beautiful skilled nursing facility located near Commack, NY is seeking a strong Staff Social Worker with a background in long-term care. This role is responsible for supporting residents and families, coordinating care planning, and ensuring compliance with regulatory requirements.
Staff Social Worker Job Description:
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
Qualifications:
- Must have BSW, MSW or LMSW
- Must have experience as a Social Worker in a skilled nursing facility
- Must have experience in long-term care
- Must have strong organizational and case management abilities
- Excellent communication and interpersonal skills
